Post COVID-19 Infection Silver Health Check Package

Our “Silver” Post COVID-19 health check is designed to focus on and screen for some of the most worrying post-covid health concerns – those affecting the lungs and the heart. 

The program includes: general health assessment, SARS-CoV-19 antibody level, complete blood count, cardiac/muscle enzymes, urinalysis, Chest X ray, ECG, and a face-to-face doctor consultation. The assessment will take approximately 30 minutes to complete and the report will be ready in 3-5 days.

Depending on your needs, our Gold and Platinum packages offer an even more comprehensive evaluation of these organ systems, plus others, following covid-19 infection. At Humansa, our aim is to help you to achieve full post-covid recovery while optimizing your health.

I have read and understood the Personal Information Collection Statement of Humansa (“PICS”).

In addition to the New World Group Privacy Policy Statement (“NWG’s Privacy Policy Statement”), the respective Privacy Policy Statement of Humansa Health (HK) Limited (“Humansa”) and/or the relevant Humansa Health Service Providers (collectively referred to as “Humansa’s Privacy Policy Statement”) and Personal Information Collection Statement of Humansa (“PICS”) (Please refer to the NWG’s Privacy Policy Statement, Humansa’s Privacy Policy Statement and the PICS, the terms set out in the PRC Addendum (“Addendum”) are applicable to:

  1. an individual located in Mainland China who uses the Application (as defined in the PICS) or visits the Website (as defined in the PICS) of Humansa and/or the relevant Humansa Health Service Providers, or otherwise provides personal data to Humansa and/or the relevant Humansa Health Service Provider by phone or any other means from Mainland China whether for making enquiries, reservations or otherwise; and/or

  2. an individual holding a Mainland China passport and/or resident identity card who visits Humansa and/or the relevant Humansa Health Service Providers’ physical centres in Hong Kong or otherwise uses Humansa’s and/or the relevant Humansa Health Service Provider’s products and/or services by phone or any other means in Hong Kong,

whose personal data are processed by Humansa and/or the relevant Humansa Health Service Providers.

For the present purpose, Mainland China refers to the People’s Republic of China excluding the Hong Kong Special Administrative Region of the People’s Republic of China (“Hong Kong”), the Macau Special Administrative Region of the People’s Republic of China and Taiwan. In addition, “Humansa Health Service Providers” means Humansa, Twilight Wood Limited, Trinity Link Limited trading as HD Endoscopy Centre, and their related companies/affiliates/subsidiaries, and “NWG” means New World Development Company Limited and its affiliated or related companies.

Before using our products and/or services or providing any personal data to us, please read and understand the NWG’s Privacy Policy Statement, Humansa’s Privacy Policy Statement, the Addendum and PICS, including but not limited to the following contents:

  1. In connection with the provision of our products and/or services, it will be necessary to collect personal data and sensitive personal data from you. For your information, under the applicable data protection laws and regulations in Mainland China, sensitive personal data refers to personal information that once leaked or illegally used, may easily lead to the violation of the personal dignity of a natural person or harm of personal and property safety. Such sensitive personal data may include identity card number (or passport number, where applicable), residential address, patient number, marital status, Log Files, Tracking Data, transaction information (including your characteristics and transaction behavior), and Medical Information (as defined in the PICS, i.e., your health data, health records, medical information, history, condition, drug history and/or other relevant information relating to your health, body and/or medical condition)

  2. Please note that the collected personal data and sensitive personal data will only be used in connection with the provision of our products and services and for the Purposes and/or Purposes For Collecting Medical Information (as the case may be) as set out in Clause 3 of the Addendum and the terms in PICS. Your personal data will be kept for no longer than necessary for the fulfilment of the Purposes and/or Purposes For Collecting Medical Information (as the case may be). For other details please refer to Clause 8 of the NWG’s Privacy Policy Statement.

  3. The collected personal data will be processed in Hong Kong and/or by third party service providers in Hong Kong and may be shared with Humansa and/or the relevant Humansa Health Service Providers as well as NWG and joint venture companies together referred to as NWG Affiliates and also our Marketing Partners for direct marketing or cross marketing as defined in and in accordance with Clause 16 of the NWG’s Privacy Policy Statement and Clauses 3 and 6 of the Addendum. Please refer to Clause 16 of the NWG’s Privacy Policy Statement and Clauses 3 and 6 of the Addendum for further details.

  4. Your personal data (including sensitive personal data) will be transferred to and/or stored in or outside Mainland China. For further details, please refer to Clause 7 of the Addendum.

This document is written in English and Chinese. If there is any inconsistency or conflict between the English and Chinese versions, the English version shall prevail.
